You agree to waive any and all objections that you may have to OHIO law as the governing law and to the sole and exclusive venue being Summit County, OHIO. If you do not agree to these terms then you must immediately cease using this Website and its services. Fit From Faith LLC’s failure to exercise or enforce any right or provision of these Terms and Conditions shall not constitute a waiver of such right or provision unless acknowledged and agreed to by Fit From Faith LLC in writing. You acknowledge and agree that you will not seek to litigate any claims against Fit From Faith LLC or any of its affiliates on a class action or representative party basis and that you shall pursue such claims solely on an individual basis.
Limitations on Actions
Unless the laws of the State of OHIO which may provide to the contrary are applicable to you, any action concerning any dispute that you may have with respect to, arising out of, or in connection with this Website must be commenced within one (1) year after the cause of the dispute first arises, and your failure to do so will permanently bar any cause of action that you may have.

Physical Activity and Nutrition Waiver
Release of Liability
You acknowledge that you are voluntarily participating in physical exercise and strenuous activities and that your use of the Fit from Faith wellness program and curriculum is being undertaken at your own risk. You assume full responsibility for all risk of bodily injury, death or damage to you. On behalf of yourself, you hereby agree to voluntarily release and discharge Fit from Faith (and our employees) from any and all claims, causes of action, and other liabilities of any kind whatsoever (whether known or unknown), which may occur as a result of your engaging in any fitness activity, nutritional and spiritual guidance throughout the program. Any activity incidental thereto, upon or within Fit from Faith whether arising from Fit from Faith or otherwise, and whether or not related to exercise, including without limitation, and bodily injury or death caused by any present or future medical condition of yours whether known or unknown. You further agree, on behalf of yourself to release Fit from Faith (and our employees) from any claim whatsoever on account of first aid, treatment or service rendered to you while participating in any fitness activity, nutritional and spiritual guidance. You acknowledge that you have carefully read this wavier and release and fully understand that it is a release of liability. You are waving any right that you may have to bring a legal action to assert a claim against us.
Member/Client/Student’s Responsibility
You should consult with your physician before starting any exercise and nutrition protocol, specifically with Fit from Faith. It is your responsibility to consult with your physician to determine if any medical condition exists and, if so, whether any such condition poses a direct threat to the health or safety of you or others. We reserve the right, however, to make the final determination in this regard. The undersigned client has read and understands the above regulations as they pertain to Fit from Faith.
Payment & Commitment Contract
All payments can be made in three installments or pay in full. In the event that you are not satisfied with the Fit from Faith program, no refund will be issued.

Fit From Faith’s commitment to you during your journey is to devote ourselves completely to doing all we can to help you discover what it means for you to live fully alive and thrive in this life. We are committed to teaching you how to create real life changes under real life circumstances with your health in the areas of nutrition, emotional wellness and spiritual wellness. It is our highest goal to create an environment in which you feel safe, seen, known, heard, and loved, thus feeling comfortable enough to be honest and transparent about all your thoughts and feelings with us as we coach you to better health.
Your Commitment
All of the lessons and handouts are purposed to help you in discovering your personal thought life and health journey. The greater your engagement with this, the greater chance you have at changing your current life.
What we expect from you is first a desire for change. If you don’t want to embrace change, there’s no need to understand what you believe and why you believe it. Without desire for change, there is no journey to go on. Every journey begins and ends. It starts in one place and ends in another. Fit From Faith will demand that you go somewhere. Where do you want to go? In order to go there you must desire change. If desire for change is not the number one desire in your heart at the moment of signing this, then our time together will not be well spent. Without change as your greatest desire you will not face the many fears and roadblocks that come up on your health journey. The lessons are designed to be a major key to help reform your thoughts are nutrition, movement, and unlock your beliefs. It’s meant to inspire you and throw you forward.
I commit to making all payments on time and understand that it is my responsibility to honor my commitment regardless of external financial choices/priorities. In the event of your failure to make payments, including as a result of declined payments, you have a 5-day grace period to update your payment information, as necessary, and make payment. If payment is not made by day 5, a $20 late fee will be applied to your account for that particular month.
Payment Options:
Individual rate pay in full $1,395 or 3 installments of $465; (this includes initial consultation, program content, optional biweekly or monthly 1:1 coaching, optional weekly group coaching)
Institutional group rate without insurance $1,095 or 1 installments of $165 and 2 further payments of $465; (this includes initial consultation, program content, optional biweekly or monthly 1:1 coaching, optional weekly group coaching)
Institutional group rate $1,095; insurance will cover partial cost of the program. I am responsible for $480 of this group rate (this includes initial consultation, program content, optional biweekly or monthly 1:1 coaching, optional weekly group coaching)
What you will be Receiving in the Program:

• Access for 24 weeks to hours of training that includes step-by-step tools and processes for getting becoming healthy and free internally.
• Access for 24 weeks to group support and encouragement through our Fit From Faith group chat space in Nudge platform.
• Access for 24 weeks to scheduled LIVE group coaching sessions via Zoom to get additional coaching and support as needed.

I understand that my life is my responsibility, and nobody can do the work for me or fix me, that I alone can affect change in my life by my choices. I knowingly, voluntarily, and intelligently decide to receive the services described above, and I knowingly, voluntarily, and intelligently assume all risks involved in the same.
